PARENTAL CONSENT & ASSENT
PARENTAL CONSENT FOR THE ASSESSMENT AND TREATMENT OF A MINOR
As the parent and/or legal guardian to consent on behalf of the minor whose name, age and date of birth I have provided in the ‘Book an Appointment’ form, I , hereby give my consent for the minor to seek assessment, counselling and/or psychotherapy as deemed advisable and/or necessary by the professional staff of graycellsmatter.com. The psychologist/therapist responsible for the care will explain to me the proposed treatment plan, the general nature and the extent of the benefits and risks, if any, involved in the treatment via phone or face-to-face meeting. This consent will b e valid until the minor reaches the age of 18 years. This consent can be revoked at any time by written notification. If I have questions relating to the consent or the proposed treatment, I may discuss them with the psychologist/therapist in charge.
ASSENT OF THE MINOR
If you are under the age of 18 years (minor), law requires the written consent of a parent or legal guardian to receive psychological assessment, counselling and treatment. Parents or legal guardians have the rights to be notified of psychological services and access to treatment records of their children/wards who are minors.
The psychologist/therapist in charge of your care may involve your parents or guardian in your services, as deemed necessary. However, before involving them in your care and giving them any information, the psychologist/therapist will discuss the matter with you, if possible and do her/his best to handle any reservations you may have with what s/he is prepared to discuss. There are some special circumstances under which you can be seen and talked to without the presence of your parents/guardian. Your psychologist/therapist will determine whether the circumstances are applicable in your situation. Every consideration will be given to you and your situation to involve your parents/guardian in the way that is most beneficial for you. Thank you for your cooperation.
